Actalent RN Care Manager Remote in St. Louis, Missouri

Job Description

Seeking a dedicated Remote Case Manager to reach out to Medicare members to enroll them in the Care Management program. The successful candidate will build care plans, complete medication reviews, and assess care plans. Responsibilities include completing 20 calls per day, evaluating member needs, facilitating care plans for optimal outcomes, developing ongoing care plans, coordinating care between members and healthcare providers, providing resource support, and educating members and their families on various health-related topics. This project extends through the end of 2024 with potential to last through 2025 or transition to a permanent role.

Hard Skills

  • RN Compact License and California RN License

  • Experience with Medicare

  • Home Healthcare/Hospice experience preferred

  • Remote Case Management

  • Proficiency in Utilization Management software (Trucare preferred)

Soft Skills

  • Strong communication skills

  • Ability to coordinate and collaborate effectively

  • Problem-solving skills

  • Attention to detail
